  • A copy of valid MHPC Registration Certificate <\/li>\n\n\n\n
  • A copy of a valid Medical Fitness Certificate issued by a competent Medical Doctor<\/li>\n\n\n\n
  • A copy of valid Security Clearance Certificate <\/li>\n\n\n\n
  • A copy of valid Citizenship Identity Card (Both sides) <\/li>\n\n\n\n
  • A copy of Valid Audit Clearance Certificate, if employed<\/li>\n\n\n\n
  •  Merit\/Other certificates (if any)<\/li>\n\n\n\n
  • One recent passport-size photograph<\/li>\n\n\n\n
